Understanding Pediatric and Adult Dysphagia: A Practical Overview for Clinicians and Caregivers

Whether you are a speech-language pathologist working across the lifespan, a parent
watching your infant struggle at the breast or bottle, or a family member helping a loved one recover from a stroke, understanding dysphagia, difficulty swallowing, is the first step toward safer, more confident mealtimes. This article breaks down the essentials: how dysphagia differs in children versus adults, what warning signs to watch for, when to seek help, and what practical strategies can make a real difference at home.
What Is Dysphagia, and Why Does It Matter?
Dysphagia refers to any disruption in the coordination and integration of the swallowing

stages that compromises the safety, efficiency, or adequacy of oral intake. It is estimated to affect roughly 8% of the world's population. In children, dysphagia can impair growth, neurodevelopment, and respiratory health. In adults, it carries significant risks for dehydration, malnutrition, aspiration pneumonia, and reduced quality of life. Regardless of age, early identification and appropriate management are critical.
Pediatric vs. Adult Dysphagia: Key Differences
Although the swallowing mechanism involves the same four phases: oral preparatory, oral transit, pharyngeal, and esophageal. The causes, presentations, and treatment approaches differ substantially between children and adults.
In children, dysphagia is often rooted in developmental and congenital factors. Common etiologies include prematurity, neuromuscular disorders such as cerebral palsy, structural anomalies of the aerodigestive tract (e.g., laryngomalacia, [cleft palate](/rare-disease/cleft-palate), laryngo-esophageal cleft), congenital heart disease, genetic syndromes, and gastrointestinal conditions like gastroesophageal reflux disease (GERD) and eosinophilic esophagitis. As many as 50% of parents report that their otherwise healthy children have some type of feeding problem, and dysphagia prevalence is particularly high among premature infants and children with complex medical histories. Importantly, the presentation of dysphagia changes as a child develops — an infant's difficulty coordinating the suck-swallow-breathe sequence looks very different from a school-age child's food refusal or a toddler's chronic cough during meals.
In adults, dysphagia most commonly arises from acquired neurological conditions. Stroke is the leading cause, with incidence estimates as high as 78% among stroke survivors. Other frequent etiologies include Parkinson's disease, traumatic brain injury, head and neck cancers, progressive neuromuscular diseases (e.g., [ALS](/rare-disease/amyotrophic-lateral-sclerosis), muscular dystrophies), and age-related sarcopenia affecting the swallowing musculature. Adult patients can often describe their symptoms — food "sticking," coughing with liquids, a sensation of incomplete swallowing — whereas young children and infants rely entirely on caregiver observation and clinical assessment.
Red Flags in Early Feeding Stages
For families and clinicians working with infants and young children, recognizing the warning signs of dysphagia early can prevent serious complications such as aspiration pneumonia, chronic lung disease, and failure to thrive. Key red flags include:
Prolonged feeding times (consistently over 30 minutes per feed in infants)
Coughing, choking, or gagging during or after feeds
Wet or "gurgly" vocal quality during or after eating
Nasal regurgitation of food or liquid
Arching, stiffening, or pulling away during feeds
Recurrent respiratory infections without another clear cause
Poor weight gain or failure to thrive
Color changes (cyanosis or pallor) during feeding
Little interest in feeding or outright food refusal
A critical concept for both clinicians and families is often referred to as “silent” aspiration. That is, aspiration that occurs without any visible coughing or distress. Silent aspiration is thought to result from decreased laryngopharyngeal sensation or neurologic weakness, and it cannot be detected by bedside observation alone. Children with recurrent lower respiratory infections and no other obvious cause should be evaluated for dysphagia, even in the absence of overt feeding symptoms.
In adults, red flags include a new wet or gurgly voice quality after swallowing, unexplained weight loss, taking significantly longer to finish meals, frequent throat clearing, coughing with thin liquids, recurrent pneumonias, and a sensation of food lodging in the throat.
When to Refer
Any child or adult displaying the red flags above warrants referral to a speech-language pathologist (SLP) for a clinical swallowing evaluation. The SLP is widely recognized as the preferred professional for evaluating and managing swallowing disorders across the lifespan.
A bedside or clinical swallow evaluation is typically the first step, but it is important to understand its limitations: bedside assessment alone cannot detect silent aspiration.
When clinical suspicion is high or the bedside evaluation is inconclusive, instrumental swallow assessment is indicated. The two gold-standard tools are the videofluoroscopic swallow study (VFSS, also called the modified barium swallow or MBS) and the fiberoptic endoscopic evaluation of swallowing (FEES). Both allow direct visualization of swallow physiology and can confirm the presence, timing, and severity of penetration or aspiration. There is no consensus favoring one over the other; clinical judgment should guide the choice based on the individual patient's needs and setting.
For children, a multidisciplinary team approach, including pediatricians, otolaryngologists, pulmonologists, gastroenterologists, neurologists, dietitians, and SLPs, is strongly recommended. In adults, particularly post-stroke, guidelines from the American Heart Association/American Stroke Association recommend that all stroke survivors be screened for dysphagia before any oral intake is initiated.
Safe Consistencies and the IDDSI Framework
Texture modification, thickening liquids and altering food textures is a therapeutic cornerstone of dysphagia management. The principle is straightforward: aspiration risk varies with bolus consistency, and modifying texture can improve swallowing safety.
Since 2017, the International Dysphagia Diet Standardization Initiative (IDDSI) has provided a globally standardized framework for describing food and liquid textures. The IDDSI framework uses a continuum of eight levels (0 through 7), each identified by a number, color code, and descriptive label. Levels 0–4 describe drinks (from thin to extremely thick), and levels 3–7 describe foods (from liquidized to regular). This framework replaces older, inconsistent terminology like "nectar thick" or "honey thick" and is now widely adopted across healthcare systems, including the U.S. Department of Veterans Affairs.
For infants and children, thickened feedings can help reduce laryngeal penetration and aspiration by slowing bolus transit and improving bolus cohesion. Thickeners must be chosen carefully based on the child's age and medical history. Options include rice cereal, carob bean, and xanthan gum-based products. For adults, thickened liquids can reduce aspiration frequency. Though, clinicians should be aware that increased viscosity may also increase pharyngeal residue and can negatively affect hydration, nutrition, and patient acceptance over time. Because of these trade-offs, texture modifications are best viewed as one component of a comprehensive, individualized treatment plan rather than a permanent solution.
Strategies for Home Use
Families play an essential role in dysphagia management. Practical strategies that can be implemented at home (always under the guidance of the treating SLP!). These include:
Positioning: Ensure the individual is seated fully upright during all meals and for at least 30 minutes afterward. For infants, maintain a supported, slightly reclined position that mimics natural feeding posture.
Pacing: Slow the rate of feeding. For infants, this may mean using a slower-flow nipple or pausing between sips. For adults, encourage smaller bites, smaller sips, and putting the utensil down between bites.
Compensatory maneuvers: The chin-tuck posture, where the chin is tucked toward the chest during swallowing, can reduce aspiration risk in many adults with stroke or degenerative disease. The head-turn maneuver, turning the head toward the weaker side, can redirect the bolus toward the stronger side in patients with unilateral weakness.
Environmental modifications: Minimize distractions during meals. A calm, focused mealtime environment helps both children and adults attend to the act of swallowing.
Consistency adherence: Follow the SLP's diet recommendations precisely. If thickened liquids or a specific IDDSI level has been prescribed, consistency is key, even small deviations can increase aspiration risk.
Oral care: Maintain good oral hygiene. Reducing the bacterial load in the mouth decreases the risk of aspiration pneumonia if aspiration does occur.
Documenting Progress and Communicating with Families
Clear, consistent documentation and open communication are the backbone of effective pediatric and adult dysphagia management. For SLPs, documentation should capture baseline swallowing function, specific findings from clinical and instrumental evaluations (including penetration-aspiration scale scores when available), the treatment plan with measurable goals, diet level recommendations using IDDSI terminology, and objective markers of progress over time, such as changes in diet level, tolerance of new consistencies, weight trends, and respiratory status.
Validated tools can help standardize documentation and track outcomes. The Functional Oral Intake Scale (FOIS) provides a seven-point ordinal scale describing the level of oral intake. The Eating Assessment Tool (EAT-10) is a patient-reported outcome measure useful for screening and monitoring symptom severity. The Penetration-Aspiration Scale (PAS) quantifies the depth and response to airway invasion during instrumental studies. Using these tools consistently across evaluations creates a clear, objective record of change.
When communicating with families, clarity and empathy are paramount. Explain findings in plain language. What is happening during the swallow, why certain foods or liquids are safer than others, and what the goals of therapy are. Demonstrate strategies in real time whenever possible. Provide written home programs with specific instructions, and revisit these at every session. Encourage families to ask questions and to report any changes in symptoms between visits. For pediatric patients especially, caregivers are integral members of the treatment team, and their observations at home often provide information that clinical sessions cannot capture.

Dysphagia is a complex, often underrecognized condition that affects individuals from the neonatal intensive care unit to the geriatric rehabilitation ward. While the etiologies and presentations differ across the lifespan, the core principles remain the same: early identification, thorough evaluation, individualized treatment, and strong collaboration between clinicians and families. Whether you are an SLP refining your clinical approach or a family member navigating a new diagnosis, understanding these fundamentals empowers better outcomes and safer mealtimes for the people who matter most.
